FREE Sports Physicals for Jefferson Student-Athletes
Jefferson Athletics is excited to once again partner with OSF HealthCare to provide FREE sports physicals for Jefferson student-athletes this spring.

These physicals are available for rising 10th-12th grade students and will cover participation for all sports seasons during the 2026-2027 school year.

This is a great opportunity for families to complete required athletic physicals at no cost while getting ready for another exciting year of J-Hawk athletics.

Important Information

A parent/guardian does not need to attend if the online permission form is completed ahead of time.

Parents/guardians are still welcome to attend if they choose.
Students should attend during their sport’s scheduled time when possible, though athletes from other sports may filter in as schedules allow.

Monday, May 18
3:30 PM — Football
5:30 PM — Boys Volleyball / Boys & Girls Basketball / Boys Soccer

Tuesday, May 19
3:30 PM — Cheer & Track & Field
5:30 PM — Baseball / Softball / Girls Soccer
